Waive UC Student Health Insurance

Please Note:
Students enrolled in 6 or more credit hours or in co-op will automatically be enrolled in the Student Health Insurance Plan, and will be charged $1,232 per semester.

STUDENTS MUST SUCCESSFULLY OPT OUT OF COVERAGE BY THE DEADLINE TO AVOID THIS FEE.

All waivers are processed online through Gallagher.  For assistance with your waiver, please call Gallagher at 1-888-538-6474.  For all other concerns, please contact the student health insurance office at 513-556-6868.


Student Health Insurance Waiver Deadline 2018 - 2019

  • Fall Semester 2018  -  09/11/2018    
  • Spring Semester 2019  -  01/29/2019  
  • Summer Semester 2019  -  05/28/2019

Requirements to Waive Student Health Insurance




In order to waive the Student Health Insurance Plan you must confirm the following:


  • I will be actively enrolled in my current Health Insurance Plan the entire time for which I am enrolled in classes.
  • My insurance is through a U.S. based insurance company, employing a U.S. based claims administrator.
  • In addition, my Health Insurance Plan:
    • Does not contain a pre-existing condition waiting period, or the waiting period has expired
    • Includes limitations and exclusions consistent with the UC Student Health Insurance policy
    • Provides access to local doctors, specialists, hospitals, and other health care providers in emergency and non-emergency situations in the UC area
    • Is fully compliant with the Affordable Care Act, including the 10 Essential Health Benefits:
      • Ambulatory patient services
      • Emergency services
      • Hospitalization
      • Maternity and newborn care
      • Mental health, substance use disorder services and behavioral health treatment
      • Prescription drugs
      • Rehabilitative services and devices
      • Laboratory services
      • Preventative and wellness services for chronic disease management
      • Pediatric services, including oral and vision care



    Steps to Waive Student Health Insurance


    1. Log into your catalyst account under www.catalyst.uc.edu
    2: Under ‘To Do’ list click ‘Student Health Insurance Waiver’

          If the waiver is no longer on your ‘To Do’ list:

    • Under ‘My Dashboard’, on the left hand side click on ‘Student -Health Insurance’
    • Select ‘Health Insurance Waiver status.  
    • You will be required to select the year you are waiving coverage for.
    • Click “continue” and you will be re-directed to the Gallagher web page.
    • (Go to step 6)

    3. Click Review and Complete.
    4. At the bottom of the page click “Waive Student Health Insurance” if you do not want the Student Health Insurance Plan
    5.You will need to check the box to agree to the terms, then click confirm, and then continue.
    6. You will be re-directed to Gallagher’s web page.  If you are not re-directed, ensure that your pop up blockers are turned off.
    7. Click on “I want to waive”
    8.You will be required to answer questions and input your information.
    9.You will be provided a confirmation number when the information is complete.
    10. You will receive two emails, one to confirm that they received the information, the second will confirm if the waiver was approved or denied.





     Student Health Insurance Waiver Denied?

       

    You will receive an email that will state the reason for the denial.  Many times additional information is required.

    If you would like to submit additional information:

    1. Log into Catalyst
    2. Under ‘My Dashboard’, on the left hand side click on ‘Student Health Insurance’
    3. Select ‘Health Insurance Waiver status.
    4. You will be required to select the year you are waiving coverage for.
    5. Click “continue” and you will be re-directed to the Gallagher web page.
    6. On the left hand side, click “View My Submitted Forms”
    7. You can now edit the data that was input, or if you need to attach a document scroll to the bottom of the screen and you will be given the option to upload documentation.
    8. You will then re-submit the form



    Check Your Student Health Insurance Waiver Status

    1. Log into Catalyst
    2. Under ‘My Dashboard’, on the left hand side click on ‘Student Health Insurance’
    3. Select ‘Health Insurance Waiver status.  
    4. You will be required to select the year you are waiving coverage for.
    5. It will state if the waiver is approved or denied. 

 

 

 

Premium Rates for 2018-2019


Single Student Coverage:  $1232 per semester for Fall and Spring Semesters ONLY (Spring coverage extends through summer). $597 Summer semester- only applicable if NOT charged in the spring
